You’ll prepare six main requirements for the Gold Eagle College Scholarship.
The Gold Eagle College Scholarship Grant Documentary Requirements include proof that you applied to or were admitted to a partner school, the scholarship form, a 2×2 photo, your Grade 11 and Grade 12 report cards, Good Moral Certificate, and family income documents.
If an ITR isn’t available, you’ll want to use InLife Foundation’s own notarized Affidavit of Source of Income template.
Overview
Requirements to Prepare
Your Gold Eagle application starts with proof that you took an exam for, applied to, or were admitted to one of InLife Foundation’s partner schools.
Before You Prepare the Documents
A couple of basics to check first:
- You’re an incoming first-year college student.
- You’re applying to or enrolled in an eligible course at one of InLife Foundation’s six partner schools.
You’ll also want to meet the scholarship’s grade, income, enrollment, discipline, and other-scholarship rules. You’ll find the complete list further down, under Qualifications.
Documents You’ll Need
InLife Foundation asks for these six main requirements:
- Proof of examination, application, or admission to one of the six partner schools
- Duly accomplished Scholarship Application Form
- 2×2-inch photo with white background
- Certified True Copy of your Grade 11 and Grade 12 Report Cards with GWA
- Certificate of Good Moral Character
- Family income documents
- Certified True Copy of the latest BIR Income Tax Return or ITR; or
- Notarized Affidavit of Source of Income if an ITR isn’t available
Your income requirement may involve documents from more than one family member, so it’s worth reading that section carefully.
What Each Requirement Is For
What each document is mainly for:
| Requirement | What it helps show |
|---|---|
| School examination/application/admission proof | That you’re applying to or have been admitted to an InLife Foundation partner school |
| Scholarship Application Form | The information InLife Foundation asks from you |
| 2×2 photo | Your identification photo |
| Grade 11 and Grade 12 Report Cards | Your academic performance and GWA |
| Good Moral Certificate | Your conduct record |
| ITR / Affidavit of Source of Income | Your family’s financial situation |
Proof of Examination, Application, or Admission
Your first step is to prepare proof that you:
- took the admission examination for;
- applied to; or
- were admitted to
one of InLife Foundation’s six partner schools.
You’ll submit this through the initial Gold Eagle form.
If you pass the initial check, InLife Foundation will send you the link to the full Scholarship Application Form.
Scholarship Application Form
You don’t need to look for the full scholarship form before completing the first step.
Once InLife Foundation confirms that you can proceed, you’ll receive the link to the Scholarship Application Form.
It’s worth filling it out carefully and checking your information before you submit it.
2×2 Photo
You’ll need a 2×2-inch photo with a white background.
A clear photo, along with any additional instructions shown in the scholarship form, will make this easy to get right.
Grade 11 and Grade 12 Report Cards
You’ll need Certified True Copies of both your Grade 11 and Grade 12 Report Cards with your General Weighted Average or GWA.
For Grade 12, your available grades should cover at least:
- the first term; or
- the second grading period.
You won’t need to wait for your entire Grade 12 school year to finish, as long as you already have the minimum grades InLife Foundation asks for.
Certificate of Good Moral Character
You’ll need a Certificate of Good Moral Character.
The scholarship page doesn’t give a special format or name a specific school officer who must sign it.
You can follow your school’s usual process, unless InLife Foundation gives you another instruction.
Latest ITR or Affidavit of Source of Income
You’ll need income documents for the family members covered by InLife Foundation’s rule.
If an ITR is available, you’ll want a Certified True Copy of the latest BIR Income Tax Return.
If an ITR isn’t available, you’ll want a notarized Affidavit of Source of Income.
InLife Foundation asks you to use its own affidavit template:
It’s best to stick with this template rather than another affidavit format, since InLife Foundation notes that other formats won’t be accepted.
Whose Income Documents Do You Need?
You’ll want income documents for:
- both of your parents;
- your legal guardian, if applicable; and
- your siblings who are working.
Each person covered by the rule should have an applicable ITR or the required notarized Affidavit of Source of Income.
This means you may need several income documents rather than just one for your household.
Where to Get the Requirements
| Requirement | Where you can get or prepare it |
|---|---|
| School examination/application/admission proof | Your partner school or its admission system |
| Scholarship Application Form | InLife Foundation sends the link after your initial check |

| Grade 12 Report Card | Your Senior High School |
| Good Moral Certificate | Your Senior High School |
| ITR | Applicable tax records of your parents, guardian, or working siblings |

| Report Cards | Depends on your school |
| Good Moral Certificate | Depends on your school |

| Affidavit of Source of Income | Depends on completing and notarizing the required form |
If you pass the initial check, InLife Foundation notes that you should receive the link to the full scholarship form within two business days.
If you send your initial response after 4:00 PM, on a weekend, or on an official holiday, it’ll be treated as received on the next business day.
How Much Do the Requirements Cost?
InLife Foundation doesn’t give one total cost for preparing all your documents.
| Requirement | Cost |
|---|---|
| Initial form | No application fee is stated |
| School proof | Depends on your school |
| 2×2 photo | Depends on how you have it prepared |
| Report Cards | Check with your school |
| Good Moral Certificate | Check with your school |
| ITR | Depends on whether your family already has the required copies |
| Affidavit of Source of Income | Notarial cost may apply |
Checking the current fee with the school, office, or notary before you pay is always a safe bet.
How to Prepare the Requirements
1. Check Your Partner School and Course
Before requesting documents, it helps to confirm:
- your school is one of the six Gold Eagle partner schools; and
- your course is included in the list for that school.
The eligible course list is different for each partner school.
2. Prepare Your School Proof
Get the document showing that you:
- took the admission examination;
- applied to the school; or
- were admitted.
Use whichever proof fits your situation.
3. Submit Your Initial Gold Eagle Form
Submit your school proof through:
Gold Eagle Initial Application Form
InLife Foundation checks this before sending you the full Scholarship Application Form.
4. Complete the Scholarship Application Form
If you can proceed, InLife Foundation will send you the link.
Fill in the information requested, and it’s worth reviewing your answers before you submit.
5. Prepare Your 2×2 Photo
Use a 2×2-inch photo with a white background.
6. Request Your Grade 11 and Grade 12 Report Cards
Ask your school for Certified True Copies.
It helps to check that:
- your Grade 11 record is complete;
- your Grade 12 record covers the required grading period;
- your GWA can be seen; and
- both copies are properly certified.
7. Get Your Good Moral Certificate
Request your Certificate of Good Moral Character from your school.
It’s worth checking your name and other important details before you submit it.
8. Check Who Needs an Income Document
Review whether you need an income document for your:
- mother;
- father;
- legal guardian, if applicable; and
- working siblings.
It’s best to prepare a document for every family member covered by the rule, not just one parent.
9. Prepare the ITR if Available
For each family member covered by the rule, prepare a Certified True Copy of the latest BIR ITR if available.
10. Use InLife Foundation’s Affidavit if There’s No ITR
Download the required template:
Complete it using the correct income information and have it notarized.
It’s best not to swap this out for another affidavit format.
11. Review Everything
Before you finish your submission, it helps to check that:
- your school is covered;
- your course is covered;
- your school proof is correct;
- your Scholarship Application Form is complete;
- your 2×2 photo has a white background;
- both Report Cards are certified;
- your Grade 12 record covers the minimum grading period;
- your Good Moral Certificate is included;
- each family member who needs an income document has one; and
- any affidavit you use follows InLife Foundation’s template.
12. Keep Your Own Copies
Keeping copies of everything you submit is always a good habit.
Common Requirement Mistakes to Avoid
Trying to Complete the Full Scholarship Form First
Your first step is to submit proof of examination, application, or admission.
InLife Foundation sends you the full Scholarship Application Form after checking your initial submission.
Assuming Every Course at a Partner School Is Covered
Your school may be a partner, but your course still needs to appear on that school’s eligible-course list.
Submitting Report Cards That Aren’t Certified
You’ll need Certified True Copies of both your Grade 11 and Grade 12 Report Cards.
Waiting for Your Entire Grade 12 School Year to Finish
Your Grade 12 grades only need to be available through at least the first term or second grading period for this requirement.
Preparing Only One Parent’s Income Document
It’s worth checking the income requirement for:
- both parents;
- your legal guardian, if applicable; and
- working siblings.
Using a Different Affidavit Format
If an ITR isn’t available, use InLife Foundation’s Affidavit of Source of Income template.
Other formats aren’t accepted.
Forgetting to Have the Affidavit Notarized
The alternative income document needs to be a notarized Affidavit of Source of Income.
Using the Wrong Family Income Limit
Your income limit depends on which partner school you’re attending.
For:
- UP Diliman, your parents’ annual gross income shouldn’t exceed ₱500,000.
- Provincial partner SUCs, your parents’ annual gross income shouldn’t exceed ₱250,000.
